Inadequate A & E Response Leads to Patient's Death

Inadequate A & E Response Leads to Patient's Death

A failure by a junior doctor to seek specialist advice left Oliver without appropriate treatment for his heart problems.

Having been suffering significant chest pains for a day, Oliver decided to attend a walk-in centre for medical attention.

At the walk-in centre his symptoms were considered to be a cause for concern and the results of an ECG led the practitioners to consider that he might be suffering a myocardial infarction or heart attack. They therefore arranged for Oliver to be taken by ambulance to an Accident and Emergency facility.

A junior doctor at Accident and Emergency examined Oliver and arranged another ECG. The results of the original ECG do not appear to have been available to the doctor and, when the new examination appeared normal, he assumed that the first ECG had been incorrect. Further tests led the doctor to conclude that Oliver's symptoms were not cardiac in nature. Unfortunately, he did not seek the advice of a more senior doctor.

Oliver was discharged without arrangements being made for any further investigation despite the fact that his symptoms would usually indicate a heart problem. Nor was Oliver advised that he should return to Accident and Emergency if his symptoms deteriorated.

Oliver continued under the care of his own GP for the management of his blood pressure and cholesterol.

Sadly, Oliver's symptoms had been indicative of a heart problem and Oliver died suddenly three and a half months after his attendance at hospital. His death was attributed to heart disease.

Had he received more thorough care and been referred for specialist investigation, it is likely that Oliver's condition would have been identified and treatment undertaken that would have prevented his early death.

Oliver's widow approached Glynns for support in investigating the quality of her husband's care and our medical experts found that it had been inadequate.

She was awarded compensation in excess of £80,000.
