The BBC recently reported on a shocking example of an NHS ‘never event’, a mistake in a medical procedure which should never happen.
In this particular incident a young woman who had attended an outpatient clinic for fertility treatment was mistaken for another patient. Instead of the anticipated procedure, she underwent a colposcopy, an invasive investigation of the cervix, normally carried out in regard to cervical screening.
NHS ‘Never Events’ include occurrences such as a procedure being carried out to the wrong patient or the wrong part of the body, as well as an incorrect procedure being carried out, incorrect medication being administered or surgical equipment being left inside a patient following surgery.
The incident, identified in a safety report by the Healthcare Safety Investigation Branch is one of 472 serious incidents in the 2019 to 2020 period.
Mistakes of this nature within the NHS vary in nature and severity but can cause permanent and debilitating damage both physically and psychologically.
