The devastation of fournier's gangrene

A delay in the treatment of fournier's gangrene can be utterly devastating to the individual affected, traumatic both psychologically and physically and destructive to the perineal, genital and anal region.

Fournier's gangrene is a form of necrotising fasciitis which is specific to the perineal region. Necrotising fasciitis is an infection of the deep soft tissue which causes the decay of all the tissue which becomes infected and which requires the debridement of all infected tissue as the only way to prevent the further spread.

Both the infection and its treatment can be destructive to the patient's physical integrity and plastic surgery is often required to restore the patient's body in so far as this is possible. The requirement to remove all tissue can mean that the patient has to undergo numerous surgical procedures until this is achieved.

The physical impact alone can be significant. The patient may struggle to work, to walk and to attend to their personal care. They may experience considerable pain and discomfort and, if the bowel and anal region is affected, may require the fitting of a stoma on either a temporary or permanent basis.

They may need to adapt their home to meet their new physical needs. They may need personal help to manage their daily hygiene chores.

Diagnosis and misdiagnosis

A prompt and accurate diagnosis of this appalling infection is crucial to the patient's outcome. Necrotising fasciitis is a medical emergency. Where there is any uncertainty whatsoever, the relevant medical professional should seek a second opinion or refer their patient for specialist assessment. A failure to do so may result in a misdiagnosis.

The likely symptoms of intense pain, swelling, discolouration of the skin as well as tenderness, possibly in the region of a wound or cut should trigger the possibility of a severe soft-tissue infection as the explanation.

A misdiagnosis will delay specialist intervention, will most likely lead to a significant worsening of the patient's condition and outcome, and may even threaten their life.
