- A failure to carry out an appropriate examination. For women who have given birth through the vagina, such an examination would include a digital rectal examination to assess the condition of the anal muscles
- A failure to identify damage to the internal or external anal muscles or both
- A failure to make a correct diagnosis of any damage to the anal sphincter such as diagnosing a 3c tear as a 3a tear or diagnosing a 3rd degree tear as a 2nd degree tear
- A failure to provide an appropriate and comprehensive repair, leading to the development or continuation of symptoms which would not have been the case with an acceptable repair
