4th degree tears are the most severe and damaging form of vaginal birth tear. They commence at the vagina, travel across the perineum, through both the external and internal anal muscles and even damage the lining of the anal canal.
They pose a risk of the new mother developing a fistula between the anus and the vagina, meaning that she may experience leakage from the vagina.
In some cases, the fitting of a stoma is necessary to provide bowel function to the woman affected.

The impact can be overwhelming. The new mother can find herself struggling to cope with totally unanticipated and distressing bowel symptoms whilst trying to nurture and support her new-born baby. The challenges can be huge, fundamentally undermining her confidence, affecting her personal relationships and her ability to work and restricting the tasks she can manage and the activities which she can engage in.
The psychological impact can also be difficult to manage.
Making a claim for compensation
Where negligent maternity care has contributed to or caused this appalling situation, a successful claim would address issues such as those mentioned above.
If the new mother had planned to return to work but is now unable to do that or can only work restricted hours due to her debilitating bowel problems, her loss of income would be included in a claim. This would apply to past losses since the birth of the baby as well as anticipated future loss of earnings.
If specialist treatment or equipment is necessary in order to help the new mother manage her symptoms, the costs of this would also be included in the claim.
If additional support or help is needed by the new mother because of the disabling nature of her injury, the associated costs would be included.
