An OASI (obstetric anal sphincter injury) in childbirth is a known risk. Because of this, it should be entirely possible to ensure that any woman who is unfortunate to suffer such an injury receives both a diagnosis and a comprehensive repair.

It should not be the case that a new mother goes home with an unrepaired severe birth tear and struggles with shocking bowel problems for the rest of her life.

An OASI can sometimes occur as a baby is born and it can vary in extent and impact. It may only affect the external anal muscle (3a and 3b tears) or it may additionally damage the internal muscle and lining of the anal canal (3c or 4th degree tear respectively).

OASI negligence

All women who have given birth through the vagina require a digital rectal examination to check for any damage to the anal sphincter.

If a severe tear has occurred, it needs to be diagnosed accurately as a 3a, 3b, 3c or 4th degree tear.

The woman should then undergo surgery in an operating theatre where her injury is effectively repaired in full by an experienced surgeon.

If this process does not occur or if a misdiagnosis is made, the injury will not be repaired and the new mother will return home, not only with her baby, but with highly distressing and disabling bowel problems of incontinence of both wind and faeces.
