A 4th degree tear during vaginal childbirth is the most severe form of maternal perineal tear. Substandard medical care may justify a claim for compensation for the trauma experienced as a result.

A 4th degree tear which occurs during childbirth causes extensive damage and appalling symptoms.

The tear, which commences at the vagina, causes injury to both the skin and the muscle of the perineum and continues through the external and internal anal sphincter. It additionally causes damage to the internal lining of the anal canal. The anal sphincter is a combination of muscles which control and prevent the passing of wind and faeces. Damage to these vital muscles will mean that the new mother will start to experience urgency when needing to go to the toilet. In some cases, she may suffer from leakage or incontinence.

The internal damage of a 4th degree tear can also allow a fistula to develop where an abnormal passage exists between the rectum and vagina allowing wind and faeces to leak from the vagina.

When a woman has given birth through the vagina, it is important that she is examined for signs of anal sphincter damage.
