There is a big difference between a woman who has suffered a 1st degree tear and one who has suffered a fourth degree tear, damaging her perineum and both her external and internal anal muscles.

If a thorough examination does not take place, however, it is probable that any damage will not be accurately identified. In these circumstances, the woman may not receive a full and effective repair and may suffer on-going symptoms of incontinence, urgency and pain.
